Active Referral Scheme

The Active Referral Scheme provides professional support to inactive adults with health conditions and comprises a number of different intervention programmes. The aim is to get people active in a way that is safe, effective, and sustainable. 

Programmes are delivered by specialist exercise instructors who provide support tailored to participant’s health status.

Exercise Referral Scheme (L3)

Exercise Referral programmes provide a 12-week course of physical activity supervised by Exercise Referral Instructors (ERIs). You will start with an initial consultation including discussion of your health conditions, targeted goal setting activities, and a programme induction. If you are on a leisure centre based programme, you will be able to access 12 consecutive weeks of sessions. These will take place at set times and there will be a mixture of gym-based and studio-based activity. If you are working remotely with an ERI, they will work with you during a telephone consultation to plan your physical activity over the next few weeks. They may offer you home based exercise, or sign-post you to exercise sessions in the community.

You will have a review session with an ERI after 6 weeks, and again after 12 weeks to discuss your progress towards your goals and make any changes as needed. At the end of the 12 weeks, we will encourage you to continue exercising regularly in any way that you can. You will be sent a follow-up questionnaire 6 months and 12 months after starting in order to assess the long-term impact of this programme.

The leisure centre programmes take place at Enderby Leisure Centre and Huncote Leisure Centre.

Am I eligible?

The eligibility criteria are as follows:

  1. Currently inactive (<30 minutes of physical activity per week).
  2. Committed to increasing physical activity levels long-term, not just for a 12-week intervention.
  3. Living with one or more stable, controlled long-term health conditions.

Your healthcare practitioner can send a referral on your behalf by emailing exercise.referral@blaby.gov.uk.

Cardio-Pulmonary Rehabilitation

C-P Rehab provides a 16-week course of physical activity supervised by Cardio-Pulmonary Rehab Instructors (CPRIs). It is aimed at those recovering from a recent cardiac/pulmonary event or surgery, and those with a moderate-severe cardiac/pulmonary condition.

You will start with an initial consultation including discussion of your health conditions, targeted goal setting activities, and a programme induction. After your induction you will be able to access 16 consecutive weeks of sessions. The types of sessions that will be available may be gym-based, class-based, or both.

There will be two sessions per week available during the daytime. You will be invited to a review session with an instructor after 8 weeks, and again after 16 weeks to discuss your progress towards your goals and make any changes as needed.

At the end of the 16 weeks, we will encourage you to continue exercising regularly in any way that you can. You will be sent a follow-up questionnaire 6 months and 12 months after starting in order to assess the long-term impact of this programme.

This programme takes place at Enderby Leisure Centre and Huncote Leisure Centre.

Referrals usually come from the Phase 3 team at the hospital, and should be emailed to exercise.referral@blaby.gov.uk.

Back Pain

The Back Pain Programme takes place as a block of 10-12 classes at a small cost.

Details for programmes in 2021 are yet to be confirmed. You may express your interest by e-mailing leisure@blaby.gov.uk.

Positive Activity Referral Scheme

The Positive Activity Referral Scheme aims to provide a sport and physical activity pathway for young people and families engaged with support services to improve self-efficacy, activity levels and reduce anxiety towards sport and physical activity whilst using self-help resources to maximise the likelihood to an active lifestyle.

The initial 12-week programme provides support and guidance for both the individual and their family to access sport and physical activity within the community.
